Body Image I am unhappy with the way my body looks. I have been thinking more and more about gastric or cosmetic surgery I don’t exercise regularly because I am ashamed of the size and shape of my body. I am uncomfortable being naked with my partner/ lover. I have engaged in numerous dieting or exercise programs to reshape my body. I avoid activities such as swimming and sex because I am uncomfortable/ unhappy with my body I find myself saying negative things about my body to myself and/ or to my friends How I feel about my body negatively affects my mood and/ or my level of anxiety. I spend a disproportionate amount of time each day with thoughts and behaviors that are body-focused.
Weight I wish I weighed less than I do. The number on the scale determines how I feel about my body and how I see myself in the mirror. I would rather be thin than healthy. I sometimes/ often skip meals in hopes that it will help me lose weight. I have been on numerous diets because I want to lose weight. I have lost weight on diets/ programs but I eventually have gained back that weight. My life would change for the better if I lost weight. Genetics are responsible for my weight and size. I believe that my weight issues are caused by my lack of will power and self-control. I believe that all women can be thin if they really work hard at achieving that.
Eating I often overeat and then feel stuffed and guilty. I sometimes/ often wonder if I have an eating disorder. If I overeat, I then restrict my food to compensate. After I have eaten, I often think, “Why did I eat that?” I eat a great deal of high calorie, high sugar and high fat foods. As a part of “my diet” I have eliminated certain types of foods. I believe that there are only two categories of foods, good and bad. I believe that if I eat when I am hungry and what I want, I will get fat. I am afraid of food and eating. I need help with making healthy and nutritious food choices.

Add up all of yesses. If you have at least 15, you more than likely have some challenges with body image, weight, and food. If you scored 20 or more yesses, you definitely need to work on your understanding, beliefs, and behaviors in these categories.
